HIGH INTENSITY SUPPORTS FOR COMPLEX CARE MANAGEMENT

FEATURED: Wollongong Harbour

HIGH INTENSITY SUPPORTS.

We provide structured, consistent support for NDIS participants with complex behaviours and high support needs.

This includes participants where:

  • behaviours of concern are present

  • support has become unstable

  • multiple providers have not worked

  • consistency and structure are missing

HOW WE WORK.

We follow behaviour support plans and work alongside:

  • Behaviour Support Practitioners

  • Support Coordinators

  • Allied Health teams

Support is adjusted based on what is actually working—not assumptions.

WHAT THIS LOOKS LIKE IN PRACTICE.

Support is built around:

  • consistent staffing teams

  • clear daily routines

  • predictable environments

  • practical, real-world skill development

We don’t rotate staff unnecessarily.

We focus on stability first—then progress.

WHEN TO REFER TO APEX.

We are typically brought in when:

  • behaviours are escalating

  • services have broken down

  • consistency has been an issue

  • a higher level of structure is required

If support is already stable and low intensity, we may not be the right fit.

SUPPORT WE PROVIDE.

  • Assistance with daily living (including high support needs)

  • Community access and engagement

  • Behaviour support plan data collection and collaboration

  • Support in high-risk or escalating environments

  • Structured routines and skill development
